I need the following information:
Name
Address
Phone
Email
City
State/Province
Postal Code
Country
Another possibility is that you can use another computer and go to
online activation page and fill in the form using your request
number form the offline computer, and then type the resulting activation
code back into your other computer and click Activate Now.
